Primary care services
North Country HealthCare provides extensive primary care services to low income, uninsured, or underinsured people in northern Arizona. As the only publicly supported community health center in the area, our charges are based on a sliding fee scale determined by income and household size. North Country offers full-scale outpatient primary healthcare in Flagstaff, Lake Havasu Ash Fork, Seligman, Grand Canyon, Kingman, Winslow, Holbrook, St. Johns, Round Valley and Show Low.

Appointments
In order to see the provider of your choice at a time convenient for you, please call our office to make an appointment. Patients can also be seen on a "walk-in" basis, however appointments are encouraged to minimize your wait. To schedule an appointment please refer to the phone listing of locations for the clinic location nearest you.

Walk-Ins
If you suddenly become ill, our providers will make every effort to see you on a walk-in basis. Walk-ins with children will be seen first.

After Hour Emergencies
There is always an on-call provider available by phone. Please call your main clinic number and our answering service will contact a provider about your concern.

Payment for services
North Country HealthCare accepts private insurance, AHCCCS, Medicare and offers a sliding fee scale for financially eligible persons. We also accept most major credit cards as well as care credit for payment.

Sliding fee scale
North Country provides quality healthcare to people regardless of their ability to pay. If you are uninsured and financially eligible, our sliding fee scale allows you to pay variable rates, with a $25 minimum charge per visit. All sliding fee scale patients are required to supply income verification for the last 30 days (a recent paycheck stub or, if self employed, a copy of last year’s tax return), identification, proof of residency (a utility bill), dependent information and a current AHCCCS denial letter based on your current income.

Private health insurance
Please check with our front office staff to determine if your health insurance is one of the plans we accept. Please notify us of any secondary insurance coverage. Any insurance co-pay amounts should be made at the time of your visit. You will be responsible for your bill, including the deductible or any other amount your insurance carrier does not pay.

AHCCCS
North Country accepts Health Choice, Phoenix Health Plan IHS, Evercare and Mercy Care Plans offered by AHCCCS. Patients not already on AHCCCS who appear eligible may apply with our AHCCCS eligibility department. You will need identification, your social security card, birth certificate, income verification for the last 30 days and proof of residency. If you are accepted by the AHCCCS program, they are able to cover more of your medical expenses than we can. If you are denied medical coverage, bring a copy of the current denial letter to verify that you complied, so we can continue to serve you on a sliding fee basis.

KidsCare Program
KidsCare is a state sponsored health insurance program for uninsured children under the age of 19. North Country is a KidsCare provider through Phoenix Health Plan or Health Choice. If your children are eligible, they will be referred to KidsCare by our eligibility workers.

Payment arrangements
If you are concerned about paying your bill, please talk to someone in our billing department who will work out a convenient payment plan for you. Unless prior arrangements are made, payments are expected on the day the service is performed. You may pay by cash, personal check, money order, care credit, MasterCard, Visa, American Express, or Discover.

Follow-up care

New prescriptions
North Country HealthCare has a pharmacy at its Flagstaff location. At other North Country locations your provider will fax your prescriptions to the Flagstaff based pharmacy. Turn around of your prescription occurs in 3-5 business days working best for medications used on a monthly basis. Payment can be completed at your clinic or through a credit card on file at the Flagstaff Pharmacy.

Prescription refills
Call the pharmacy 928.213.6103 at any time to leave a message by pressing 1, with your name and prescription numbers. Press 2 to speak to a member of the pharmacy team.

Need a refill but none are available?
Call the pharmacy approximately 10 days before your prescription is gone. We will contact your provider for further refills. The provider’s office or the Flagstaff Pharmacy will notify you if no refills are authorized.

Lab results
If your lab results are abnormal, your provider will call you as soon as they receive the results. If the results are normal, a letter will be sent to you. If you need to discuss your results at length, please make a follow-up appointment with the same provider.

Services

Dental services
North Country houses an up to date dental clinic to serve the needs of our patients. We accept most major insurance plans and will work with you if you are uninsured as well. Children receive dental services on a sliding fee scale basis, while qualified low-income adults are charged as little as 50% of the normal fee. Behavioral Health Services
North Country offers Integrated Behavioral Health Services. Clients that want to make lifestyle changes to improve health, manage their health challenges and diagnoses, or are experiencing stress or life transitions can access brief and/or on-going counseling. Pharmacy
North Country’s on-site pharmacy offers prescription discounts to their patients. A clinical pharmacist is available for consultations in diabetes, anticoagulation and complex medication regimens. For more information about our pharmacy service please call

Outreach and Training
North Country also provides preventive and wellness programs such as the Well Woman HealthCheck Program, Colorectal Cancer Screening Program, Maternal Child Health, Prenatal and Health Start Program, HIV/AIDS Program, NACASA (Northern Arizona Center Against Sexual Assault) RESEP (Radiation Exposure Screening & Education Program) and a Diabetes Program.
