Diabetes Education & Self-Management Program


North Country HealthCare is a participant in the Bureau of Primary Healthcare’s Diabetes Collaborative. The Diabetes Education & Self-Management Program strives to provide primary care and disease management services to all diabetic and pre-diabetic patients across the North Country HealthCare service area. The program also offers assistance to individuals and their families with Type I Diabetes, Type II Diabetes, Gestational Diabetes and Pre-Diabetes. 

Qualifications? Eligibility?

Any individual who has been diagnosed with Type 1 or Type 2 Diabetes is eligible to be a part of the Diabetes Collaborative at North Country HealthCare. Education is also provided for the following; Gestational Diabetes (diabetes during pregnancy), weight loss, weight management, nutrition counseling, self-management of blood sugar, glucometer teaching, and more. Therefore any individual in need of these services or forms of education can receive them through the North Country HealthCare Diabetes Program.  

 

The recommended Standards of Care for patients with diabetes and provided at North Country HealthCare include:

  • case management services
  • diabetes education
  • retinal scans
  • behavioral health counseling
  • annual depression screening
  • annual diabetic foot exams
  • instruction & support in self-management of blood glucose
  • low-cost diabetic dental screenings
  • on-site Hemoglobin A1c testing
  • annual blood work

 

Future plans for the Diabetes Education & Self-Management Program include the incorporation of a series of four week diabetes education courses at the following sites: Flagstaff, Winslow & Kingman.

Some key topics of the courses include:

  • how food effects your blood sugar
  • identifying different sources & types of carbohydrates
  • portion size & portion control
  • healthy eating tips for healthy living
  • identifying target blood sugar values and strategies to maintain those values
  • the role of exercise in lowering blood sugar
  • medications and their purpose
  • the role of insulin

North Country offers quarterly special events called Diabetes Days. Diabetes Days are a

one-stop comprehensive visit in which a diabetic patient receives all the Standards of Care over this 2-3 hour appointment. Health professionals at a Diabetes Day include: a doctor, pharmacist, registered dietitian, ophthalmologist, dental hygienist, podiatrist, behavioral health counselor and a health educator.

Necessary lab work is also completed during this clinic visit.
