Welcome and thank you for choosing North Country HealthCare. You’ve made a great decision and we are excited to build a long lasting relationship with you. By choosing North Country HealthCare as your patient centered medical home you will have access to a variety of integrated services.

North Country HealthCare is the primary community health center in the area. In 1991, a small group of doctors and nurses, health and human service representatives and consumers worked together to establish the Flagstaff Community Free Clinic. In 1996, the clinic transitioned from a volunteer organization into a state and federally funded community health center. Today, North Country HealthCare serves 11 communities across Northern Arizona, providing quality, evidence based, medical care to all while building healthier communities.
